Permission Forms
Permission forms can be found for an event in the Youth Center in the foyer of the church and MUST be turned in by the due date to be able to attend. If there has been no response by the due date the event is subject to cancellation.
If there are no forms or if you have any questions, please call the Youth Office @ (972) 682-3585.
Please turn forms in to the Youth Office, in the Pastoral Center (old rectory).
